The Impact of Occlusal Splint Therapy and Vitamin D on the Management of Temporomandibular Disorders

Abstract
Objective: The purpose of this study is to evaluate the Vitamin D supplementation as an adjuvant therapy for stabilization splint in patients exhibiting temporomandibular disorders (TMD).
Materials and Methods: This study involved 100 TMD divided into two equal groups each of 50 patients; group 1: treated by occlusal splint plus vit D and group 2: treated by vitamin D only. All patients in adult age, more than 18 years. All patients were accepted without regard to their financial situation, caste, gender, or religion. Less than 30 ng/mL of vitamin D was present, the teeth were either completely dentate or had adequate occlusal stops, and no more than two posterior teeth (apart from third molars) were absent from each quadrant. Patients report experiencing clicking, difficulty opening their mouth, jaw pain or discomfort, and headaches as symptoms of TMD. To determine the serum levels of calcium and vitamin D, venous blood samples were taken from each member of the two groups using the normal procedures of laboratory analysis. Patients with vitamin D levels below 30 ng/mL were included. When the mouth closes, the anterior bite splint in group 1 will cause the posterior teeth to become dislocated. Data were analyzed by ANOVA tests. Also, Mann-Whitney U and Kruskal Wallis tests were used (α=.05).
Results: Splint therapy plus vitamin D (group 1) showed more improvement than vit D alone (group 2) with statistically significant difference (Pst, 3rd and 6th months post-treatment. On the other hand, serum vitamin D was increased in both groups, more in group 1 without significant difference (P>.05).
Conclusions: When used as an adjuvant therapy for TMDs vitamin D is crucial. Within a few months, occlusal splint therapy and vitamin D supplements can effectively cure temporomandibular joint issues.
